กลุ่มวิจัยเลเซอร์วิทยาทางทันตแพทยศาสตร์ มหาวิทยาลัยขอนแก่น โดยทุนอุดหนุนการวิจัยและนวัตกรรมจากสำนักงานการวิจัยแห่งชาติ ประจำปีงบประมาณ 2563 ได้ดำเนินการศึกษาวิจัยพัฒนาเลเซอร์สำหรับใช้ส่งเสริมสุขภาพช่องปากผู้สูงอายุในระดับปฐมภูมิ โดยมีวัตถุประสงค์หลัก คือ เพื่อการพัฒนาระบบการบริการทางการแพทย์ โดยการขับเคลื่อนระบบบริการสาธารณสุขให้ผู้สูงอายุที่อยู่ห่างไกล สามารถเข้าถึงบริการส่งเสริมสุขภาพช่องปากโดยใช้เทคโนโลยีที่ทันสมัย คือ เลเซอร์ โดยใช้กระบวนการวิจัยแบบมีส่วนร่วมตั้งแต่ หารือ พัฒนาเครื่องเลเซอร์ความเข้มต่ำที่เหมาะสมกับการใช้งานส่งเสริมสุขภาพช่องปากในระดับปฐมภูมิ ทดสอบเครื่องเลเซอร์ต้นแบบ และศึกษาเรียนรู้การใช้งานในพื้นที่ซึ่งมีวิธีการศึกษาโดยศึกษาประสิทธิผลทางคลินิก ร่วมกับการศึกษาเชิงคุณภาพ ในเครือข่ายจำนวน 4 เครือข่ายได้แก่ โรงพยาบาลส่งเสริมสุขภาพตำบลทาขุมเงิน โรงพยาบาลส่งเสริมสุขภาพบัวเงิน โรงพยาบาลบรบือ และโรงพยาบาลตะกั่วป่า ผลการศึกษา มีผู้ป่วยร่วมโครงการ 10 คน ทันตแพทย์ 7 คน ทันตาภิบาล 4 คน ผู้บังคับบัญชา 4 คน และภาคส่วนเอกชน 2 บริษัท พบว่า เลเซอร์ต้นแบบสามารถห้ามเลือดจากแผลถอนฟัน และจากการขูดหินน้ำลายในกรณีที่มีเหงือกอักเสบมาก ห้ามเลือดจากแผลการตัดเนื้อเยื่อส่งตรวจ ส่งเสริมการหายของแผลในช่องปากและอาการแสบร้อนเนื้อเยื่อช่องปาก ได้ประสิทธิผลดีเช่นเดียวกับผลการศึกษาวิจัยทางคลินิกที่ผ่านมา ซึ่งผู้ป่วยก็รับรู้และยอมรับถึงประโยชน์โดยตรงของการใช้เลเซอร์ส่งเสริมสุขภาพช่องปาก ทันตแพทย์ผู้ปฏิบัติใช้งานเครื่องเลเซอร์ต้นแบบได้อย่างสะดวกและให้การบริบาลผู้ป่วยด้วยความมั่นใจ ผู้บังคับบัญชาให้การยอมรับและสนับสนุนแนวคิดในการพัฒนายกกระดับการดูแลสุขภาพของผู้สูงอายุด้วยเทคโนโลยีเลเซอร์ที่ทันสมัย สรุปว่า การใช้เลเซอร์ที่ได้รับการพัฒนาทั้งเครื่องและกระบวนการที่เหมาะสมกับการใช้งานในโรงพยาบาลส่งเสริมสุขภาพตำบล และระดับอำเภอ สามารถช่วยส่งเสริมสุขภาพช่องปากผู้สูงอายุได้ดี สมควรได้รับการขยายผลในวงกว้างเพื่อส่งเสริมคุณภาพชีวิตของผู้สูงอายุคำสำคัญ (Keyword) เลเซอร์ สุขภาพช่องปาก ผู้สูงอายุ โรคทางระบบ<br><br>The Lasers in Dentistry Research Group, Khon Kaen University was granted by The National Research Council of Thailand in the year 2021 to conduct the project of laser for oral health promotion in primary care. The main objective was to improve a medical service via enhancing opportunity for rural older people to access the advanced technology, in this case, the laser for oral health promotion. The participatory action research was a main design of this project. It was composed of developing the prototype of low intensity laser for use in primary oral health care, verification of the prototype, clinical study and qualitative study in the networks. There were 4 health care services namely, Tha Khum Ngoen Sub-District Health Promoting Hospital, Bua Ngoen Sub-District Health Promoting Hospital, Borabu Hospital and Takua Pa Hospital The results from 10 patients, 7 dentist-users, 4 dental nurses, 4 hospital administrators of the networks and 2 representatives from private sectors showed that the prototype laser was usable for hemostasis of the extraction, scaling of the severe gingivitis and soft tissue incisional biopsy, promotion of healing and pain relief of the oral ulcers and tissue inflammation. These efficacies were similar to the outcomes from the reference clinical trials. The older patients accepted and realized the benefit of this laser to oral health. The laser prototype was user friendly. The dentists who used these lasers gained more confidence for treating the patients. The administrators accepted and supported this laser technology for enhancing the older patients&rsquo; quality of life. In conclusion, the prototype of laser and process of technology transferring which were developed for primary and secondary health care, benefited oral health promotion in older people.
