เชื้อแบคทีเรียดื้อยาคาร์บาพีเนม โดยเฉพาะแบคทีเรียตระกูล Enterobacterales เป็นเชื้อดื้อยาที่องค์การอนามัยโลกและประเทศไทยให้ความสำคัญในการเฝ้าระวังอยู่ในลำดับแรก การตรวจคัดกรองหาเชื้อดื้อยาคาร์บาพีเนม จึงมีความจำเป้นต่อการฝ้าระวัง การศึกษานี้ได้ปรับปรุงและพัฒนาชุดทดสอบเพื่อตรวจคัดกรองหาแบคทีเรียดื้อยาคาร์บาพีเนม รวมถึงจำแนกชนิดเชื้อ Escherichia coli, Klebsiella/Enterobacter และ Salmonella spp. ได้ในชุดทดสอบเดียว ชุดทดสอบที่ได้สามารถใช้ทดสอบและแปลผลได้ตรงตามชนิดเชื้อที่ได้ทดสอบ คือ E. coli ให้ผลการทดสอบเป็นสีน้ำเงิน Salmonella spp. ให้ผลการทดสอบเป็นสีน้ำตาลแดง ในขณะที่ Klebsiella และ Enterobacter ให้ผลการทดสอบเป็นสีเหลือง ชุดทดสอบมีค่าความไว ค่าความจำเพาะ ค่าพยากรณ์ผลบวก ค่าพยากรณ์ผลลบ และค่าความแม่นยำ เท่ากับ 98.1%, 99.7%, 99.5%, 98.7% และ 99% ตามลำดับ มีค่าสัมประสิทธิ์ความสอดคล้อง (Cohen’s Kappa coefficient) เท่ากับ 098 หมายถึงอยู่ในเกณฑ์ความน่าเชื่อถือดีเยี่ยม ในขณะที่ค่า major error และ very major error อยู่ที่ 0.34% และ 1.95% ตามลำดับ การทดสอบชุดทดสอบในระดับภาคสนามกับโรงพยาบาลระดับจังหวัดจำนวน 5 แห่ง ให้ค่าความแม่นยำสัมพัทธ์ ความไวสัมพัทธ์และความจำเพาะสัมพัทธ์ ดังนี้ 96.5%, 96.2% และ 96.7% ตามลำดับ ชุดทดสอบสามารถเก็บรักษาและคงความสามารถในการทดสอบได้ถึง 10 เดือน ณ อุณหภูมิ 4 และ 25 องศาเซลเซียส ในรูปแบบชุดทดสอบแห้ง โดยมีราคาต้นทุนอยู่ที่ 62.35 บาทต่อ 1 ชุดทดสอบ สำหรับการทดสอบเชื้อจำนวน 1 ตัวอย่าง<br><br>Carbapenem-resistant bacteria, especially Enterobacterales is an important first issue for public health surveillance by the World Health Organization and Thailand. Development of screening test for carbapenem-resistant bacteria is necessary for this surveillance. In this study, we modified and developed a screening test to determine resistance to carbapenem and distinguish Escherichia coli, Klebsiella/Enterobacter, and Salmonella spp., in a single test by revealed blue, yellow, and red-brown colors, respectively. This screening test showed 98.1% of sensitivity, 99.7% of specificity, 99.5% for positive-predictive value, 98.7% of negative-predictive value, and 99% of accuracy. Reliability of the test is moderated agreement level by Cohen’s Kappa coefficient is 0.98. In addition, major error and very major error were 0.34% and 1.95%, respectively. Application of this test to three hospital laboratories revealed relative accuracy, relative sensitivity, and relative specificity of 96.5%, 96.2%, and 96.7%, respectively. This screening test could be storage in dry format for up to 10 months in refrigerator (4-8 oC) or at room temperature (25oC). Unit cost of the screening test is 62.35 Thai Baht per 1 test.
